Continuity of Care Specialist – Guiding Successful Transitions Beyond Treatment
As a Continuity of Care Specialist, you play a vital role in ensuring smooth, well-coordinated transitions for individuals moving from treatment back into the community. You’ll lead the discharge planning process from start to finish—collaborating closely with clinical teams, facility staff, and external partners to create personalized aftercare plans that support long-term recovery and stability.
With a focus on communication, coordination, and compassionate follow-through, you’ll serve as the critical link between in-facility care and community-based support—empowering individuals served to continue their progress with confidence.
Discharge Planning: Responsible for creating and implementing discharge plans, ensuring adherence to policies and procedures.
Stakeholder Relationship Management: Develop and maintain relationships with internal and external stakeholders, including treatment teams and community agencies serving individuals with mental health or serious medical conditions.
Financial Assistance and Benefits: Facilitate applications for financial assistance and help individuals resume benefits as part of the discharge process.
Life Skills and Support Activities: Provide individual and group activities to enhance coping, life skills, and support for individuals upon release.
Continuity of Care Coordination: Coordinate medical and medication needs upon release, assist with housing, and make appointments with mental health agencies to ensure continuity of care.
