The Recovery Plan Coordinator (RPC) plays a key role in supporting individuals’ recovery by coordinating, organizing, and facilitating their treatment and rehabilitative services. Working closely with a team lead, the RPC ensures that each individual’s Recovery Plan is created and maintained with care, aligning with facility protocols and guidelines. This position is central to ensuring that treatment plans are not only documented but actively contribute to the success and well-being of those served. It's an opportunity to make a real impact on the recovery journey of individuals in a supportive, team-driven environment!
Recovery Planning Coordination: Coordinates the recovery planning process, including the development and documentation of the Recovery Plan, ensuring that it is comprehensive and up to date.
Treatment Team Leadership: Schedules and directs Treatment Team meetings, ensuring coordination of participation from community case managers and relevant professionals.
Recovery Plan Updates and Monitoring: Initiates, reviews, and updates Recovery Plans as needed, ensuring services are tailored to changes in the individual's functioning and progress.
Advocacy and Service Implementation: Works with and advocates for individuals served to ensure personalized services are implemented effectively, fostering an individualized approach.
Collaborative Service Delivery: Collaborates across disciplines to improve facility service delivery, ensuring a holistic approach to patient care and maximizing outcomes.
